STAT News, a medical trade publication, reported in June 2026 that publishing in The Lancet costs $9,550 on the open access route. Manusights, an editorial-services tracker, gives a figure of around $6,500. Both cite primary experience with recent submissions.
Why the numbers differ
Elsevier uses personalised APC pricing for Lancet titles — the exact amount shown during production varies by country, institution and applicable agreements. Two authors submitting to the same Lancet journal in the same week can see two different prices in production. The list price is the ceiling.
The subscription route is free
The Lancet remains a hybrid publisher. If you skip the open access option, publication costs the author nothing — the article sits behind a paywall for 12 months, then some titles release freely. For unfunded authors, this is a legitimate route.
